Job Description

Location: Glendale, AZ Pay Rate: $60/hour Schedule: Full-Time, 40 hours/week (Monday-Friday) Position Type: In-Person About the Role We are seeking a dedicated Physical Therapist to join our team. The Physical Therapist will conduct evaluations, develop individualized plans of care, and provide therapy services in alignment with physician orders and professional standards. Responsibilities + Perform evaluations and develop treatment plans for clients. + Deliver and document therapy services according to established plans of care. + Collaborate with families and other professionals to support client success. + Ensure compliance with all federal, state, and local regulations. Minimum Requirements + Current Arizona Physical Therapist License. + Graduate of an accredited Physical Therapy program. + One (1) year of prior professional experience preferred. + Current CPR certification (if applicable). + TB questionnaire, PPD, or chest X-ray (if applicable). + Current health certificate (per contract or state regulation). + Must meet all federal, state, and local requirements. + Must be at least 18 years of age. Caseload Details + Caseload split between 2-3 locations, all within a 15-20 minute radius of central Glendale, AZ. + Specific caseload details will be discussed during the interview.
