Type: Travel Contract (13 Weeks)
Shift: Day Shift, 5x8-Hour (07:30 AM 4:00 PM), Variable Weekend Coverage
Providence Health and Services in Medford, OR is seeking a skilled and compassionate Licensed Clinical Social Worker (LCSW) to join their Social Work Case Management team. In this vital role, you'll support patients with complex psychosocial needs including mental health assessments, behavioral health placements, and social determinants of health (SDOH) navigation.
This is an excellent opportunity for licensed clinicians who are passionate about making a meaningful impact in an acute care setting , while collaborating with a dynamic interdisciplinary care team.
Responsibilities:Conduct comprehensive risk and psychosocial assessments across hospital units.
Develop and implement discharge plans and provide clinical social work services.
Coordinate mental health and substance use disorder (SUD) placements.
Provide patients with access to community resources related to SDOH.
Collaborate with physicians, nurses, and case managers to ensure coordinated care.
Float to other units as needed to meet staffing demands.
Work variable weekends based on departmental PTO coverage.
License: Active Oregon LCSW license (must be active at time of submission no exceptions).
Experience: Minimum 2 years of recent clinical social work experience in a hospital or acute care setting.
Caseload: Comfortable managing caseload ratios of 1:40 .
Systems: Proficient in Epic EMR charting.
Travelers: First-time travel clinicians welcome.
Other: Experience with risk assessments, psych/SUD placements, and resource coordination is required.
Education: Master's Degree in Social Work (MSW) from an accredited institution.
Certifications: Active Oregon LCSW license; BLS not required.
